Important: Represented Care is a directory, not a healthcare provider. Nothing on this site is medical advice, and we do not endorse or guarantee any provider. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Agreement to These Terms
These Terms of Service ("Terms") are a legally binding agreement between you and Represented Care LLC ("Represented Care," "we," "us," or "our"). They govern your access to and use of representedcare.com and any related pages, features, and services we offer (together, the "Services").
By accessing or using the Services, creating an account, or purchasing a paid plan, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Services. If you use the Services on behalf of a practice, clinic, or other organization, you agree to these Terms on its behalf and confirm you have authority to do so.

1. Our Services
Represented Care is an online directory that helps people find Black healthcare providers in the Atlanta area. Searching and viewing the directory is free for patients and other visitors. Healthcare providers and practices can create and manage listings, either on our free Basic plan or on an optional paid plan.
We provide a way to discover providers. We do not provide healthcare, schedule or coordinate care, or take part in any relationship between a patient and a provider.
2. Medical Disclaimer
Represented Care is not a healthcare provider, and nothing on the Services constitutes medical advice, diagnosis, or treatment. Information on the Services, including provider listings, is for general informational purposes only and is not a substitute for the judgment of a qualified healthcare professional.
Any relationship, communication, or care between a patient and a provider is solely between those parties. We are not responsible for the services any provider delivers, the outcome of any care, or any fees a provider charges.
IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM IMMEDIATELY. DO NOT USE THE SERVICES FOR EMERGENCIES.
3. No Endorsement or Verification
Provider listings are created and maintained by providers. We do not endorse, recommend, or guarantee any provider, and the appearance of a provider on the Services is not a recommendation by us.
We do not guarantee the accuracy, completeness, or currency of listing information, including licensure, credentials, education, specialties, insurance accepted, languages spoken, fees, location, or availability. We may, but are not required to, review listings or check certain information. Any review we perform is limited, reflects a single point in time, and is not a guarantee.
You are responsible for independently confirming a provider's qualifications and suitability before seeking care, including by checking with the appropriate state licensing board and with your insurance plan.
4. Eligibility
You must be at least 18 years old to use the Services. A parent or legal guardian may use the Services to find care for a minor in their care. The Services are not directed to children under 13, and we do not knowingly collect personal information from them.
5. Accounts
Some features, including creating and managing a provider listing, require an account. When you create an account, you agree to:
- provide accurate, current, and complete information and keep it up to date;
- keep your login credentials confidential and not share your account with anyone outside your practice who is not authorized to manage it;
- notify us promptly at connect@representedcare.com if you suspect unauthorized access to your account; and
- accept responsibility for all activity that occurs under your account.
We may refuse to create an account, or may suspend or close one, as described in these Terms.
6. Provider Listings
If you create or manage a listing, you represent and agree that:
- you are the provider named in the listing, or you are authorized by that provider or practice to create and manage the listing on their behalf;
- you and any listed provider hold all licenses, certifications, and registrations required to provide the services described, and you will update or remove the listing promptly if that changes;
- all information in the listing, including credentials, specialties, insurance accepted, pricing, and availability, is accurate and not misleading, and you will keep it current;
- you have the rights to any photos, logos, or other materials you upload; and
- your listing and your practice comply with all laws that apply to you, including professional licensing, advertising, and patient privacy laws.
We decide, in our discretion and consistent with our listing criteria, whether a listing may be published on the Services. We may edit listings for formatting, request supporting information, or decline, hide, or remove any listing that we believe is inaccurate, violates these Terms, or is otherwise inappropriate for the directory.
7. Plans, Billing, and Cancellation
Plans
Providers may list on our free Basic plan, which is free with no time limit and is not a trial. Providers may also choose a paid Standard or Premium plan, billed either monthly or annually. Current features and prices for each plan are shown on the Services at the time of purchase. We may change the features included in the Basic plan at any time.
Payment
Prices are in U.S. dollars and do not include any applicable taxes, which you are responsible for. We accept Visa, Mastercard, American Express, and Discover. Payments are processed by our payment processor, Stripe, and your payment information is subject to Stripe's terms and privacy policy. We do not store your full card number. You authorize us, through Stripe, to charge your payment method for all fees for the plan you select.
Automatic renewal
Paid plans automatically renew at the end of each billing period (monthly or annually, according to the option you chose) at the then-current price, and your payment method will be charged automatically, unless you cancel before the renewal date.
Cancellation
You may cancel a paid plan at any time by logging into your account and managing your plan, or by emailing connect@representedcare.com. Cancellation takes effect at the end of your current paid period. Your paid features remain active until then, after which your listing automatically moves to the free Basic plan.
Refunds
Fees are non-refundable, and we do not provide refunds or credits for partial billing periods, unused time, or downgrades, except where required by law.
Failed payments
If a payment fails, we may retry the charge and may notify you to update your payment method. If payment is not completed, we may move your listing to the free Basic plan.
Price changes
We may change plan prices. We will give you advance notice by email before a price change applies to you, and the new price will take effect at your next renewal. If you do not agree to the new price, you may cancel before it takes effect.
8. Paid Plans and Placement
Providers on paid plans may receive enhanced profile features or increased visibility within the directory. Paid placement is not an endorsement or recommendation by Represented Care. Payment does not affect the content, display, or removal of user reviews.
9. Your Content
"Content" means anything you submit to the Services, including listing information, photos, descriptions, and, when available, reviews and responses to reviews. You keep ownership of your Content.
By submitting Content, you grant us a non-exclusive, worldwide, royalty-free, transferable, and sublicensable license to host, store, reproduce, display, adapt (for example, to fit formatting or layout), and distribute your Content in connection with operating, improving, and promoting the Services. This license continues for a reasonable period after you remove the Content or close your account, to allow us to remove it from our systems and backups, and continues indefinitely for any reviews you posted that remain visible on the Services, as described below.
You are solely responsible for your Content. You confirm that you have the rights needed to submit it and that it does not violate these Terms, any law, or anyone else's rights. We are not responsible for Content submitted by users, and we may remove or restrict any Content at any time, for any reason, without notice. We are not obligated to monitor Content, but we may do so.
Do not post protected health information. Do not post private medical details about any patient or other person, including in a listing, a review, or a response to a review.
10. Reviews
When review features are available, the following rules apply:
- Reviews must reflect your own, genuine, firsthand experience with the provider you are reviewing.
- You may not review yourself, your own practice, a practice where you work, or a competitor's practice, and you may not ask anyone else to post a review on your behalf.
- No one may offer, request, or accept payment, discounts, gifts, or other incentives in exchange for a review, or for changing or removing a review.
- Reviews must not contain protected health information, threats, harassment, hate speech, or statements you know to be false.
- Providers may respond to reviews professionally, but must not disclose any patient's health information or confirm that a reviewer was a patient.
We may remove reviews that violate these Terms or the law. We do not remove or suppress reviews because they are negative, and payment for a paid plan does not affect which reviews appear. If you believe a review violates these Terms, contact us at connect@representedcare.com.
11. Prohibited Activities
You agree not to:
- misrepresent your identity, licensure, credentials, specialties, or affiliation with any healthcare provider or practice;
- create a listing for a provider or practice you are not authorized to represent;
- post false, misleading, or unverifiable information in a provider listing, including inaccurate insurance, pricing, or availability details;
- post reviews of a provider you have not personally received care from, or reviews of your own practice or a competitor's practice;
- offer, request, or accept payment, discounts, or other incentives in exchange for reviews;
- post protected health information or other private medical details about any patient or other person;
- use the Services to solicit patients or users for purposes unrelated to healthcare services;
- sell, rent, or otherwise transfer your account or listing to anyone else;
- scrape, copy, or compile provider listings or user data from the Services, whether manually or by automated means (including bots, crawlers, or scrapers), for any commercial purpose or to build a competing directory or database;
- use any information obtained through the Services to harass, threaten, stalk, or harm any person, or to send unsolicited marketing;
- impersonate any person or entity, or access or attempt to access another user's account;
- upload viruses or other harmful code, or interfere with, disrupt, overload, or attempt to gain unauthorized access to the Services or the systems that run them;
- circumvent or disable any security, access, or usage-limiting feature of the Services;
- copy, modify, decompile, or reverse engineer any part of the Services' software, except as permitted by law;
- post anything unlawful, defamatory, harassing, hateful, obscene, or that infringes anyone's intellectual property, privacy, or other rights; or
- use the Services in violation of any applicable law or regulation.
12. Copyright Complaints
We respect intellectual property rights. If you believe material on the Services infringes a copyright you own or control, email connect@representedcare.com with the subject line "Copyright Complaint" and include:
- your physical or electronic signature;
- identification of the copyrighted work you believe is infringed;
- identification of the material you believe is infringing and where it appears on the Services, with enough detail for us to locate it;
- your name, mailing address, telephone number, and email address;
- a statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law; and
- a statement, under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on the owner's behalf.
We may remove or disable access to material that is the subject of a complaint, and we may terminate the accounts of users who repeatedly infringe others' rights. Knowingly false complaints may result in liability for damages.
13. Third-Party Sites and Services
The Services may link to or rely on third-party websites and services, such as provider websites, online booking pages, and our payment processor. We do not control and are not responsible for third-party sites or services, their content, or their privacy practices. Your use of them is at your own risk and subject to their terms.
14. Google Maps
If and to the extent the Services include Google Maps features and content, use of those features and content is subject to the then-current versions of the Google Maps/Google Earth Additional Terms of Service and the Google Privacy Policy. By using the Services, you agree to be bound by these Google terms.
15. Our Intellectual Property
Except for user Content, the Services and everything in them, including the Represented Care name and logo, site design, text, graphics, software, and the selection and arrangement of directory content, are owned by or licensed to us and are protected by intellectual property laws.
Subject to these Terms, we grant you a limited, non-exclusive, non-transferable, revocable license to access and use the Services for your personal use or, if you are a provider, for managing your listing and your practice's internal business purposes. You may not use our name, logo, or other marks without our prior written permission, except that listed providers may state that they are listed on Represented Care. All rights not expressly granted are reserved.
16. Feedback
If you send us suggestions, ideas, or other feedback about the Services, you agree that we may use it for any purpose without restriction or compensation to you.
17. Privacy
Our Privacy Policy explains how we collect, use, and share personal information. By using the Services, you acknowledge that your information will be handled as described there.
18. Changes to These Terms
We may update these Terms from time to time. When we do, we will update the "Last updated" date at the top of this page. If we make material changes, we will also notify account holders by email before the changes take effect. For material changes that affect paid plans, we will give at least 30 days' notice. Your continued use of the Services after updated Terms take effect means you accept them. If you do not agree to updated Terms, you must stop using the Services and may cancel any paid plan.
19. Changes to the Services
We may change, add, or remove features of the Services, or suspend or discontinue the Services, at any time. We do not guarantee that the Services will be available at all times or free of errors, and we may need to interrupt them for maintenance or other reasons. If we permanently discontinue the Services, we will provide a prorated refund of any prepaid fees for the unused portion of your paid period.
20. Suspension and Termination
You may stop using the Services and close your account at any time by contacting us. We may suspend or terminate your account, remove your listing or Content, or restrict your access to the Services at any time if we believe you have violated these Terms or the law, if your use creates risk or possible legal exposure for us or others, or if we are required to by law. If we terminate a paid plan because you violated these Terms, you will not receive a refund.
If your account is terminated, you may not create a new account without our permission. Sections of these Terms that by their nature should survive termination will survive, including those covering Content licenses for posted reviews, disclaimers, limitation of liability, indemnification, governing law, and the time limit to bring claims.
21. Disclaimers
THE SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, SECURE, OR ERROR-FREE, OR THAT ANY INFORMATION ON THE SERVICES, INCLUDING PROVIDER LISTINGS AND REVIEWS, IS ACCURATE, COMPLETE, OR CURRENT.
WE ARE NOT RESPONSIBLE FOR THE CONDUCT, QUALIFICATIONS, OR SERVICES OF ANY PROVIDER OR USER, AND ANY CARE YOU RECEIVE FROM A PROVIDER YOU FIND THROUGH THE SERVICES IS AT YOUR OWN RISK AND SOLELY BETWEEN YOU AND THAT PROVIDER.
22. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, IN NO EVENT WILL REPRESENTED CARE OR ITS OWNERS, EMPLOYEES, OR AGENTS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR RELATING TO THE SERVICES OR THESE TERMS, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
TO THE FULLEST EXTENT PERMITTED BY LAW, OUR TOTAL LIABILITY FOR ALL CLAIMS ARISING OUT OF OR RELATING TO THE SERVICES OR THESE TERMS WILL NOT EXCEED ONE HUNDRED U.S. DOLLARS ($100).
Some jurisdictions do not allow certain warranties to be disclaimed or certain damages to be limited, so some of the above may not apply to you. Nothing in these Terms limits liability that cannot be limited under applicable law.
23. Indemnification
You agree to defend, indemnify, and hold harmless Represented Care and its owners, employees, and agents from any claims, losses, liabilities, damages, and expenses, including reasonable attorneys' fees, arising out of or relating to: (a) your use of the Services; (b) your Content, including any listing you create or manage; (c) your violation of these Terms or any law; (d) your violation of anyone else's rights; or (e) if you are a provider, any care or services you provide to any patient.
24. Governing Law and Disputes
These Terms are governed by the laws of the State of Georgia, without regard to its conflict-of-law rules.
Before filing a claim, you and we each agree to first try to resolve any dispute informally by sending a written description of the dispute to the other party (to us at connect@representedcare.com) and allowing 30 days to resolve it.
If the dispute is not resolved, it must be brought exclusively in the state courts located in Cobb County, Georgia, or the United States District Court for the Northern District of Georgia, and you and we consent to the personal jurisdiction of those courts. Either party may bring an eligible claim in small claims court instead.
25. Time Limit to Bring Claims
To the extent permitted by law, any claim arising out of or relating to the Services or these Terms must be filed within one (1) year after the claim arises. Claims not filed within that time are permanently barred.
26. Electronic Communications
By using the Services, you consent to receive communications from us electronically, including by email and through notices on the Services. You agree that all agreements, notices, disclosures, and other communications we provide electronically satisfy any legal requirement that they be in writing, and that clicking to accept or using the Services has the same effect as a signature.
27. General Terms
- Entire agreement. These Terms and our Privacy Policy are the entire agreement between you and us about the Services.
- Severability. If any part of these Terms is found unenforceable, the rest remains in effect, and the unenforceable part will be enforced to the maximum extent permitted.
- No waiver. Our failure to enforce any part of these Terms is not a waiver of our right to do so later.
- Assignment. You may not transfer your rights under these Terms without our consent. We may transfer ours, including in connection with a merger, acquisition, or sale of assets.
- Events beyond our control. We are not responsible for delays or failures caused by events beyond our reasonable control.
- Relationship. Nothing in these Terms creates a partnership, joint venture, employment, or agency relationship between you and us, or between us and any provider.
